Athletics at Collegium
We are excited to announce that Collegium Athletics will be expanding athletics in the 2024-25 school year. This year all paperwork will be completed online. Thank you all for your support an-d Go Cougars!
Collegium currently fields interscholastic athletic teams for our students in grades 7-12. We also offer intramural sports for our students in grades K-6. This aligns Collegium with PIAA eligibility standards. Student-athletes who intend to participate on an interscholastic sports team(s) for the 2024-25 school year are required to complete the following forms on this site: PIAA Pre-participation (or recertification for multiple sports), Emergency Contact Form, Travel Verification Form.

Frequently Asked Questions
Q: Do I need to get another PIAA Physical Evaluation Form if my child has already participated in a sport during the current school year?
A: A: No, you will need to turn in a PIAA Recertification Form on FamilyID. Page 8 is to be completed by a parent or guardian. We will notify you if additional information is needed, and page 9 must then be completed by your child(ren)’s Primary Care Physician.
Q: Can my child play a sport that Collegium doesn’t offer?
A: A: Yes, If Collegium doesn’t offer a sport that your child is interested in, they can play that sport for their home district. Collegium students must have pre-approval from their Collegium Principal before they can leave early for games.
Parents must complete the online form that includes home district information: Athletic Director name and email, coach name and email address, game schedule for that sport indicating days and times the student would leave early for games.
