Athletics at Collegium

Collegium currently fields interscholastic athletic teams for our students in grades 7-12. This aligns Collegium with PIAA eligibility standards. Student-athletes who intend to participate on an interscholastic sports team(s) for the 2026-27 school year are required to complete all forms on Arbiter Registration.

We also offer intramural sports for our students in grades K-6. Intramural sports are managed by Intramural Coordinators at each building.

PIAA Sports Physicals (Arbiter Registration)

In order to participate in sports for the 2026-27 school year, all forms must be completed on Arbiter Registration (formerly Big Teams). Please DO NOT mail or turn in any paper physicals to the school. Please use the following links to access the forms more easily:

*This form must be completed by a physician after May 1st for the upcoming school year.

Please upload it to Arbiter Registration following your sports physical appointment.
All re-certifications are to be entered on Arbiter Registration, please go to your Arbiter Registration account and enter the Winter or Spring Re-certification (Depending on the season). No upload is needed, the athletic trainer will contact you if section 8 of the PIAA form or a doctor’s clearance. DO NOT simply update the original first physical of the year registration, the PIAA requires the Re-certification to be filled out for each season after the first.

Penn Medicine FREE PIAA Sport Physicals June 18th, 2026

This event is free to any athlete who will be in grades 7-12 for the 2026-2027 school year.

Frequently Asked Questions

Q: Do I need to get another PIAA Physical Evaluation Form if my child has already participated in a sport during the current school year?

A: No, you will need to turn in a PIAA Recertification Form on Arbiter Registration. Page 7 must be completed by a parent/guardian. Page 8 is to be completed by a Physician (the Athletic Trainer will contact you if Section 8 is needed)

Q: Can my child play a sport that Collegium doesn’t offer?

A: Yes, if Collegium doesn’t offer a sport that your child is interested in, they can play that sport for their home district. Collegium students must have pre-approval from their Collegium Principal before leaving early for games.

Parents must complete the online form, which includes home district information: Athletic Director name and email, coach name and email address, and the game schedule for that sport, indicating the days and times the student would leave early for games.
